上顎洞発生癌肉腫の1症例

A Case of Carcinosarcoma of the Maxillary Sinus

はじめに

　上皮性悪性腫瘍である癌腫と非上皮性悪性腫瘍である肉腫が，同一器官内に，時を同じくして発生する病態は，癌肉腫と呼ばれ稀な疾患である。頭頸部領域においては，甲状腺，喉頭などに発生例が比較的多くみられているが^1,2），上顎洞発生例は，数例をみるのみである。今回われわれは，極めて稀な上顎洞発生癌肉腫の1症例を経験したので，若干の文献的考察を加え報告する。

A 69-year-old woman presented with a complaint of painful swelling of the left cheek.

The radiological examination showed a large tumor of the left maxillary sinus with destruction of bony wall of the sinus.

Histopathological examination showed the tumor was composed of two different malignant elements, epithelial and non-epithelial.

The detailed histopathological examinations, by means of immunohistological technique, demon-strated the epithelial element was squamous cell carcinoma and the non-epithelial element was fibrosarcoma.

The patient was treated by irradiation, chemo-therapy and surgery. In spite of all therapies, the growth of tumor, especially sarcomatous part, made progress and she died after 9 months.

This report seems to be the 10 th case of carcinosarcoma and “so-called” carcinosarcoma originated in the nose and paranasal sinuses reported in Japan.
