慢性副鼻腔炎とThorn's test

　1948年HenchのCortisoneによるリウマチス性疾患に対する治療的効果の驚異的であると云う業績と相前後して，米国のThornの発表したACTH注射による末梢血液好酸球数の変動を指標とする副腎皮質機能検査法，即ちACTH 25mg注射4時間後に於ける好酸球減少率が注射前に比し，50％以上を副腎皮質特にZona fasciculataの機能健全なるものとし，減少率50％以下若しくは増加を示す時を機能不全とする方法は広く識者の注目する所となつた。

　而して其の後Long等によつてAdrenalinが脳下垂体前葉を刺戟してACTHを分泌させ，これが副腎皮質特にZona fasciculataに作用してCorticosteron型ホルモンを分泌させ，ACTHと同樣の効果を末梢血液好酸球に及ぼす事が明かにされ，ACTHの入手困難な事と相まつてAdrenalinを代用するも其の成績に差異なき事が認められるに至つた。爾来此のEinephrin testは広く使用せらるゝに至り，臨床方法の応用も盛んになつた。我々の専門領域に於ては此のTestは余り使用されていないが，私は試みに慢性副鼻腔炎患者に本法を施行し，其の成績と粘膜所見並に治癒状態を対比した所興味ある一定の成績を得たので茲に発表し諸賢の御批判を仰がんと思う。

　FURUKAWA reports the results of application of Thorn Test to 40 patients affected with chronic sinusitis. The test which consists of injecting 0.3cc of adrenalin solution subcutaneously, according to Hansel, causes shrinking and blanching of the swollen nasal mucous membranes in patients affected with allergy while, such changes are not seen in purulent sinusitis unrelated to allergic disturoances.

　1) Twenty eight cases showed positive test; 12 negative.

　2) Patients with positive test are prolonged in recovery; negatives recovered in much shorter period.
