I．はじめに

　特発性内頸動脈海綿静脈洞瘻（以下CCFと略）に対し，balloon catheterを用い，瘻孔とともに内頸動脈を閉塞し，同時に遊離自家静脈片を用いた頭蓋外—頭蓋内血管吻合により頭蓋内血行を再建し良好な結果を得た症例を経験したので報告し，さらに，術中血流測定の結果より，本手術法の有用性についても述べる．

A case with spontaneous carotid-cavernous fistula treated by balloon catheter technique combined with a extra-intracranial by-pass using a saphenous vein graft between the common carotid artery and the middle cerebral artery is reported.

A 72-year-old female was admitted with complaints of recurrent symptoms of carotied-cavernous fistula 4 months after spontaneous remission. Angiograms showed the presence left internal carotid-cavernous sinus fistula.
