Treatment of Carotid-cavernous Fistula Using a Detachable Balloon Catheter:A case report and review

I．はじめに

　内頸動脈海綿静脈洞瘻（以下CCFと略す）の理想的な治療とは，瘻孔のみの完全閉塞であり，神経組織や脳循環をいささかも損わないことであるといえる^7）．この意味で従来のCCF諸治療法の趨勢が，extra-vascularapproach^15）よりintra-vascular approach^{6,8,9,11,13,14,17）}へと変遷し，なかでもSerbinenko^18），Debrun2-5）らにより開発されたdetachable balloon catheter法が注目を集めているのは当然である．

　最近われわれも，同balloon catheterを外傷性CCF患者に用いる機会を得たので，若干の文献的考察を加えて報告する．

A case of post-traumatic carotid cavernous fistula, success-fully treated by a detachable balloon catheter, is reported.A 55-year-old housewife was admitted to the author's depart-ment on 25th August, 1980. Three months prior to admis-sion, she complained of right tinnitus following a trafficaccident and then developed right chemosis and exophtha-Imus.

A selective right internal carotid angiography revealedrapid filling of the carotid cavernous fistula.

Under general anesthesia, the patient was treated by themaneuver after Debrun.
