脊髄硬膜外Hemangiolipomaの1例

Spinal Epidurid Hemangiolipoma:A case report

I．はじめに

　全脊髄腫瘍のなかで脂肪腫は約1％から4.2％の頻度で認められるが^4,15），その大部分は潜在性脊椎披裂などの閉鎖障害（dysraphic condition）を伴っており，これらの先天奇形を伴わない成人の脊髄脂肪腫は稀である．組織学的に脂肪細胞の成分以外に，豊富な血管成分を含むものはhemangiolipomaと呼ばれる^11）．これは胸椎レベルの硬膜外に好発し，脊髄圧追症状の自然増悪，寛解を繰返すという特異な病像を呈することが多いことで知られているが^2），今まで文献上16例の報告をみるのみである．今回われわれは，約1年2ヵ月の経過で徐々に進行してきた42歳男の胸椎硬膜外hemangiolipomaの1例を経験したので，これを報告し，併せて脊髄脂肪腫，ことにhemangiolipomaについて文献的考察を加えた．

A 42-year-old male was admitted with a one year and twomonths history of hypesthesia below the epigastric regionand gait disturbance. On examination, increased ATR andPTR were bilaterally noticed with sensory disturbance belowabout Th.5 dermatome level. No cutaneous manifestationswere detected on his back. Plain x-ray films showed nospina bifida. Metrizamide mvelography showed a space-ocuppying mass at the Th. 5 level. At operation, an extraduraltumor, severely adhesived to the clura mater, was totallyremoved.
