急性脊髄硬膜外膿瘍の手術治験例

Acule Epidural Spinal Abscess:A Case of Complete Recovery by Surgical Trcatment

Ⅰ．序

　急性脊髄硬膜外膿瘍は，比較的稀な疾患であり，近年抗生物質の発達と共に，その頻度は，更に減少したといわれている^2）．しかし，適切な治療が行われなければ，重篤な神経症状を遺す点，その早期診断，早期治療が重要である．

　治療の原則は^4），早期予術による排膿と脊髄の減圧であり，これに加えて有効な抗生物質の投与である．本邦では著者らの知る限り，かかる急性例の手術による完全治癒の報告はみられない．最近著者らは外傷に続発し，典型的な症状を呈した急性胸髄硬膜外膿瘍の1例を経験し，推弓切除術と強力なで化学療法により全治せしめたので，文献的考察を加え報告する．

Acute spinal epidural abscess is found to be relatively rare and may be getting rarer since antibiotics have been widely used. An early diagnosis and prompt surgical treatment are still important to prevent severe neurolo gical deficit such as paraplegia.

A characteristic case is presented. Eighteen years old boy showed acute progression of paraplegia and rectourinary difficulty with complete spinal block at T 6-7 intervertebral space on myelography,who had suffered from multiple infectious lacerations on his face and hands two months previously.
