急性硬膜下血腫，脳内血腫に対する広範囲減圧開頭術について—手術前後の頭蓋内圧とCT変化

Decompressive Hemicraniectomy for Acute Subdural or Intracerebral Hematoma:Pre and postoperative Intracranial pressure and CT scan

I．はじめに

　高度の脳挫傷を伴う急性硬膜下血腫の手術成績は，減圧開頭術の併用の有無にかかわらず，現在なお極めて不良である^{1,2,4,5,12）}．われわれは，十数年前から片側の急性硬膜下血腫や脳内血腫症例に対し積極的に広範囲減圧開頭術^9,10）を行っているが，本法の有効性についてはいまだに議論の多いところである^3,4,12,17）．本法の主月的は，頭蓋内圧亢進に続発する天幕切痕ヘルニアによって生じる二次的脳幹障害を防止，ないし緩和せんとすることにある．しかしながら，術前に手術効果を予測して手術適応を決定することは必ずしも容易ではない．そこで，われわれは本法の有効性や限界を明らかにするために，術前の臨床症状，脳血管写所見等とともに，術後の頭蓋内圧測定結果をもとにして検討を行ってきた^14,15）．この結果，本手術をうけた症例の術後頭蓋内圧の変動は大きく3型に分類できること，またこれら3種類の圧変動が術後転帰と極めてよく相関していること等から，本法による減圧効果には，症例によって限界があることが明らかになった^14,15）．そこで今回は，特に手術前後の頭蓋内圧，および経時的CT変化にっいて検討し，今後の問題点についても皆干の考察を行ったので報告する．

Retrospective analysis of 15 cases with acute subdural hematoma and intracerebral hematoma associated with severe cerebral contusion which has undergone unilateral decompressive hemicraniectomy was carried out in an effort to demonstrate the effectiveness of this operative procedure. In this study, pre- and postoperative serial CT scan and intracranial pressure (ICP) monitored during operation were compared by dividing the cases into three groups according to postoperative ICP measured at the epidural space for 24 to 48 hours.
