The Diagnosis of Syringomyelia

I．はじめに

　syringomyelia　（またはhydromyelia）は従来よりよく知られた疾患である．以前は変性疾患の1つと考えられていたが，1958年Gardner^18）がsyringomyeliaはArnold-Chiari I型奇形と近縁疾患であり，脳脊髄液のdynamic factorによるものであることを発表して以来，この疾患の病態の解明，更には診断，治療面においても，非常な進歩がみられるようになった．しかしながらsyringomyelia腔内への液貯溜の病態，その診断および治療等のいずれについてもいまだ不明また解決されていない問題が多く，かなり稀な疾患のわりには欧米では文献も多く，研究者にも興味を持たれている疾患である．本邦ではこの疾患に関する文献は少ない．その理由ははっきりしないが，本邦ではこの疾患は少ないのかもしれない．またあまりにも知覚解離等の典型的な症状にこだわりすぎ，やや非定型的な症例に対してsyringomyeliaを念頭に置かないことも原因であろう^26）．

　syringomyeliaは早期治療が極めて重要な疾患である．そのためには早期に診断することが必要である，われわれは最近5年間に7例のsyringomyeliaを経験し手術的治療を加えた^38），今回は特に診断面について文献的考察を加えて報告する．

Seven cases are reported in which syringomyelia was confirmed by neuroradiological studies and surgical procedures.

Dissociated sensory loss was noted in four patients in this series. Peripheral paresis and muscular atrophy in arms and hands were found in all seven patients. In only two of the patients the wide spinal canal were found in the lateral view of plain cervical spine films. Assimilation of the atlas to the occipital bone was seen in two patients. Arnold-Chiari malformation was found by myelography and posterior fossa craniectomy in four patients.
