先天性第ⅩⅢ因子欠乏症に伴う頭蓋内血腫の3手術例

Three Operated Cases of Congenital Deficiency of Factor ⅩⅢ Associated with Intracranial Hematomas

I．はじめに

　先天性の凝固因子欠乏症のうち，第ⅩⅢ因子（フィブリン安定化因子）欠乏症は極めて稀であり，現在までの報告例も100例前後にすぎない^3,5,6）pp．本症では，約25％に頭蓋内出血を合併するとされるが^3），開頭術による治療が行われた症例は，いまだ数例に満たないようである^6）．最近私どもは，本症に頭蓋内血腫を合併した3手術例を経験したが，うち1例では術前診断が不明で，手術後に本症と考えられたものであった．本症のように，極めて稀な出血性素因による頭蓋内出血の場合，その診断や手術前後の管理の上で多くの問題がある．これらの症例は，今後同様の症例に遭遇した場合に多くの示唆を与えると考えられるため，若干の文献的考察を加えて報告する．

Theree operated cases of congenital deficiency of factor 【ⅩⅢ】 (fibrin-stabilizin factor) associated with intracranial hematomas were discribed and the diagno-sis, replacement therapy of the factor were discussed. Congenital deficiencey of factor 【ⅩⅢ】 is quite rare coagulation disorder and only 100 patients were reported in the literatures in which we could find only one case who had craniotomy for associated intra-cranial hemorrhage.

Case 1: A 41-year-old female with the history of unknown hemorrhagic diathesis complained of head-ache and right hemiparesis on August 2, 1980.
