I．はじめに

　急性硬膜下血腫は頭部外傷に伴って生じることが多いが，特発性の報告も散見される．一方，頭蓋内出血で発症する脳腫瘍は少なくないが，急性硬膜下血腫で発症することはまれである．今回われわれは，急性特発性硬膜下血腫の診断で開頭術を行い，術中所見より髄膜腫からの出血による急性硬膜下血腫と考えられた1症例を経験したので報告する．

A 46-year-old female was transported to our clinic 4 hours after the onset of sudden headache and transient loss of consciousness. On admission, she was drowsy with slight left hemiparesis and anisocoria (left>right). The CT scan revealed a right subdurout neurological deficit one month after the operation.

the operation, we found that the hematoma was de-rived from a small meningioma attached to the dura at the middle temporal fossa. The meningioma was 1.5cm in diameter. The patient was discharged without neuro-logical deficit one month after the operation.
