I．はじめに

　最近CTの進歩により，これを応用した定位脳手術がfunctional neurosurgeryの分野以外に，脳腫瘍のbio-psy^{6,11,17,25）}や小線源によるX線照射治療^10,29），血腫除去^{1,4,8,12,13）}などに利用されはじめている．われわれはまず基礎実験として血腫吸引に関する実験を行い，これに基づき高血圧性脳出血33例，外傷性脳出血2例に対してCTを応用し，定位的に血腫除去を行い，従来の開頭法とほぼ変わらない結果を得たので報告する．

Recent advancement of CT system provides not onlydefinite diagnosis and location of intracerebral hematomabut also coordinates of the center of the hematoma. Trialsof stereotactic evacuation of the hematoma have beenreported by some authors in the cases of subacute or chronicstages of hypertensive intracerebral hemorrhage. In thisseries, similar surgery has been performed in 33 cases ofhypertensive intracerebral hematoma including 22 cases inacute stage, and 2 cases of traumatic hematoma. Twenty-four patients were male and 11 patients female.
