Three cases of infants undergoing surgery for abnormal course of the extracranial internal carotid artery are presented along with a discussion on pathogenesis. The first two cases, a fifteen-month-old female and a two-year-old female, had identical signs of convulsive seizure, right hemiparesis and mental retardation. In both cases, angiogram revealed kinking and tortuousity of left internal carotid artery above the carotid hifurcation. P.E.G. showed severe atrophy of the left cerebral hemisphere. In the latter case, high voltage slow waves were found in left cerebral hemisphere on E.E.G.
