Ⅰ．はじめに

　頭部外傷に続いて起こる脳膿瘍は，通常受傷後3-5週間以内に発症する^2）といわれている．しかし受傷後1年あるいはそれ以上の無症状期間を経てから発症する遅発性脳膿瘍の報告も稀ではない．これらは時に頭蓋内新生物類似の症状を呈することがあり^2,9），診断に際しては慎重を要する．しかしながら外傷後30年以上を経過した後発症するものは非常に稀である．われわれは最近，頭部外傷後38年の間，通常の日常生活を経過した後に発症した脳膿瘍を経験したので，これを報告し若干の考察を加える．

We reported a case of delayed brain abscess following a penetrating gun-shot injury. The brain abscess surrounding retained intracranial fragments of bone and shell was found 38 years after head injury. The patient's only symptom had been minimal weakness of the right arm. This case was treated surgically with good results.

The long piriod of silent infection was discussd.
