脳内出血で発症した内頸動脈欠損症の1例

Agenesis of the Right Internal Carotid Artery with Intracerebral Hemorrhage: Case report

I．はじめに

　内頸動脈欠損症は，非常に稀であり頭蓋内血管性病変の際の検査により偶然見つかることが多い．その病変としては，脳動脈瘤が圧倒的に多く，その血行動態に基づく発生が論じられている^13）．今回われわれは，脳内出血で発症し血管撮影上，一側内頸動脈欠損症が見られ，出血源は小動静脈奇形が疑われた症例を経験した．これに文献的考察を加えて，本症における頭蓋底thin slice CT scanとsingle photon emission CT（以下SPECTと略す）の有用性について報告する．

Abstract

A 45-year-old woman was admitted to our depart-ment because of intracerebral hemorrhage. Neurological examination showed upward gaze palsy, right hemihy-pesthesia including face and nuchal rigidity. CT scan demonstrated a high density spot in the left quad-rigeminal plate. Postcontrast CT scan demonstrated an abnormal vessel in the left midbrain. We suspected the presence of an arterio-venous malformation (AVM) and angiography was perfomed.
