 A 86-year-old female patient, whose internal carotid artery situated just behind the posterior wall of her pharynx was reported. This kind of anatomical anomaly is generally considered to be congenital and rather frequent. In those cases, the internal carotid arteries ran and winded around the tonsillar and adenoidal tissues, and the frequent inflammations of these areas made the wall of the artery adhesive and fragile. Therefore these congenital and the acquired factors worked together and might be a source of severe bleeding during operation of the pharynx.
