視床出血—そのC．T．所見と治療

Thalamic Bleeding:its C.T. Findings and Treatment

Ⅰ．緒言

　高血圧性脳内出血のうち被殻出血の手術適応およびその術式については学会でもかなり以前より議論され，その考え方も現在では既に定説となっている感が深い．しかるに視床出血のそれについてはいまだ議論の余地が多い．視床出血の診断，血腫進展方式，あるいはそれに付随する病態の把握はいまだ十分ではないようである．その理由として脳血管撮影ではその詳細を知りがたいこともあろう．

　本論文では，著者らが昭和50年11月より使用しえたC．T．（Computed Tomography HITACHI C-T．-H250）による視床出血の診断，血腫の進展形式およびその臨床症状について要約し，C．T．のこの面における有用性と手術々式の選択について検討し，少し考察を加えたい．

From Nov. 1975 until June 1976, we have made a diagnosis of 10 cases of the thalamic bleedings with C.T. (Hitachi CT-250).

These 10 cases revealed the following results;

(1) There are three main locations of the hematoma in the thalamus.

1) anterior type (hematoma mainly located in the anterior nuclei)

2) posterior type (hematoma mainly located in the posterior nuclei).
