てんかんの成因と周生期障害

はじめに

　てんかんの合理的な治療と予防には原因の解明が不可欠であるが，半数以上の例では病因不明である。てんかんは内因と外因の相互作用の結果発症すると考えられ，とくに外因のみで反復発作を示すてんかんが生ずるかどうかについては疑問がある。しかし，従来，てんかん，とくに乳幼児のてんかんの推定原因として周生期脳障害が重視されてきた。そこで，ここではてんかんの成因としての周生期脳障害の意義について種々の側面から検討してみる。

Abstract

Perinatal brain damage is one of the most frequent known etiologic factors in childhood epilepsy and its significance has been repeatedly stressed because it is a preventable cause of epi-lepsy. Recent prospective follow-up studies dis-closed 4~10% of newborns with severe asphyxia developed epileptic seizures in association with varying degrees of neurological sequelae.
