脊髄の血管奇形

Vascular malformation of the spinal cord

はじめに

　脊髄の血管腫（angioma）は稀な疾患ではないが頻度の高い疾患でもない。1969年3月に第1例目のhemangioblastomaの全摘出手術を行なって以来1980年までにわれわれが経験した脊髄血管腫は，動静脈奇形（AVM）19例（うち2例はv. Recklinghausen氏病を合併する頸部のAVM），静脈奇形（venous angioma）1例，telangiectasia 3例，hemangioblastoma 3例などである^21）。本邦の主要病院を対象とした1978年の集計では，計114例（うち脊髄AVMは88例）となっている^16）。

　昭和49年当神経研究の進歩が脊髄血管障害を特集し著者^19）が脊髄血管奇形について総括的にまとめて以来8年が経過した。この間本邦での本疾患に関する発表がなされているが決して多くはない。今回は前回の報告との重複を避ける意味で，（1）脊髄の血管系について，（2）血管奇形の臨床におけるその後の考え方の変化，（3）最長10年に及ぶ遠隔成績より見た本疾患の治療法の選択，（4）その他の二，三の新しい知見などについて，自験例を中心に文献的考察も加え述べることにしたい。

　A total of 17 cascs with spinal arteriovenous malformation (AVM) was clinically surveyed.

　The classification of the spinal AVM after Djindjian et al. was modified and divided into 4 categories, namely (1) intramedullary-anterior supply, (2) intramedullary-posterior supply, (3) intramedullary-mixed supply and (4) extramedullary-posterior supply.
