人体剖検例による変性小脳歯状核の電子顕微鏡的観察

An electron microscopic observation on the human degenerative dentate nucleus

　最近，変性型ミオクローヌスてんかんや歯状核—赤核—淡蒼球—ルイ体萎縮症（以下両者を歯状核変性症と一括）で，小脳歯状核に病理学的にその特徴的変性像として認められる変化，すなわちやや核が偏在したり無核性であったりする神経細胞の腫大性変化とその胞体と樹状突起のまわりに拡がる嗜銀性の増強した構造の不明確な神経網（neuropil）の変化^14,15）の本態が注目されている。Anzil^1）は腫大性の神経細胞を神経細胞のgrumose degenerationと呼び，白木^18），Shiraki et al．^17）はこの全体像をgrumose degenerationとしている。

　われわれは2例の歯状核変性症で歯状核の一部を電顕用に採取し，腫大性神経細胞は神経細胞体の軸索反応に類似し，また周囲の嗜銀性の神経網はautophagosomeを含む特異な変性像を示すシナプス終末と終末前軸索の集塊よりなっていることを観察したので以下に報告し考察する。

　Degenerative dentate nuclei from 31-year-old female, clinically diagnosed as degenerative type of myoclonus epilepsy and 28-year-old male, as dentatorubro-pallidoluysian atrophy, were examined by the electron microscope. Pathologically both cases showed an abiotrophic degenerative change both in the pallidoluysian system and dentatorubral pathway. In the dentate nuclei neuronal depopulation was severe and survived neurons appeared inflated or swollen with slightly eccentrically-located nuclei and dendrites showing an increased argylophilia.
