脊髄運動ニューロンの病変に関する最近の知見

Recent advances in the pathology of spinal motor neurons

はじめに

　下位運動ニューロンを中心とした系統変性疾患としてのmotor neuron discaseにはその中心的なものとして筋萎縮性側索硬化症（amyotrophic lateral sclerosis ALS）があり，進行性球麻痺（progressive bulbar palsy），脊髄性進行性筋萎縮症（spinal progressive muscular atrophy，SPMA），痙性脊髄性麻痺（spastic spinal paralysis，SSP）などがその中に含まれる。以下これらの疾患の病理，とくに脊髄運動ニューロンの病変につき述べ，さらにneuronal storage disease，実験的下位運動ニューロン障害などにおける前角細胞変性についての最近の知見についても言及する。

　The recent investigations on the pathology of the spinal motor neuron were reviewed, including classical amyotrophic lateral sclerosis (ALS), familial ALS, Guamanian ALS, spinal progressive muscular atrophy, neuronal storage diseases, and experimental studies on spinal motor neurons.

　In classical ALS, the degeneration of the anterior horn cells is rather simple and shows few specific changes in light microscopy.
