膀胱の炎症性偽腫瘍の1例

Inflammatory Pseudotumor of the Urinary Bladder: A Case Report

　68歳，女性。肉眼的血尿を主訴に受診。膀胱鏡，CTにて膀胱頂部に腫瘍を認め，TURを施行した。病理組織上，炎症性偽腫瘍が最も疑われたが，横紋筋肉腫との鑑別も確定できず，膀胱部分切除術を施行した。病理組織学的，免疫組織学的検査により最終的に炎症性偽腫瘍と診断した。不必要な拡大手術を避けるべく，本症の認識が必要である。

A 68-year-old women was admitted to our hospital with a two-week history of macrohematuria and suprapubic pain. Cystoscopy and CT showed a nonpapillary, broad based tumor located at the dome of the urinary bladder. Histology of transurethral biopsy material revealed atypical spindle cells in a scanty background stroma, raising the suspicion of an inflammatory pseudotumor, with the dif-ferential diagnosis including a rhabdomyosarcoma.
