多房性嚢胞状腎細胞癌の1例

MULTILOCULAR CYSTIC RENAL CELL CARCINOMA : A CASE REPORT

　症例は41歳の男性。健診で顕微鏡的血尿を指摘され受診した。腹部超音波検査，CT検査で右腎下極に多房性嚢胞状腫瘤を認め，血管造影検査では悪性所見を否定できなかったが，病変は限局した小腫瘤のため右腎部分切除術を施行した。病理組織学的には多房性嚢胞状腎細胞癌であった。偶発的に発見された小さな多房性嚢胞状腫瘤に対しては腎保存手術を原則とすべきと考える。

A 41-year-old man presented with occult blood in urine which was found incidentally. Abdominal ultrasonogra-phy and CT scan revealed a multilocular cystic mass in the lower pole of right kidney. Although renal angiogra-phy showed a possible malignant tumor, partial ne-phrectomy was performed because the tumor was local-ized and small in size.

Histopathologically, the cystic renal mass was a multiocular cystic renal cell carcinoma. If an incidental multilocular cystic mass of the kidney is localized and small, nephron sparing surgery should be indicated.
