多房性腎嚢胞に合併した腎細胞癌

RENAL CELL CARCINOMA WITH MULTILOCULAR RENAL CYSTS:A CASE REPORT

　49歳，男性．左腎の腫瘤の検査目的で当科受診．超音波検査の結果，左腎下極に直径10cm大の多房性，嚢胞性腫瘤を認めた．造影CTでは一部嚢胞壁が増強されたが，血管造影では嚢胞壁に腫瘍血管を認めなかった．悪性の可能性のある多房性腎嚢胞の診断のもとに部分切除術を施行した．永久標本にて嚢胞壁に腎細胞癌の合併が認められた．我々は多房性腎細胞癌の診断は手術以外には困難であると思われた．

A 49 year-old male was admitted to our hospital for the examination of the left renal mass. Ultrasonogra-phy demonstrated a multilocular cystic tumor in the left kidney. Contrast enhanced computerized tomogra-phy showed the partially enhanced wall in the cystic lesion, however, selective renal angiography indicated avascularity. Under the diagnosis of multilocular renal cyst with possible malignancy, partial nephrectomy was performed. Finally histological examination of the specimen revealed renal cell carcinoma.
