嚢胞状腎細胞癌6例の臨床的検討

Cystic Renal Cell Carcinoma:Report for 6 Cases

　嚢胞状形態を呈した腎細胞癌6例について検討した。画像所見により4例が嚢胞壁より突出する腫瘤を認め，2例では隔壁を有する嚢胞壁が不均一に肥厚し造影効果を認め，腎細胞癌と診断された。5例で根治的腎摘除術、1例で腎部分切除術を施行した。病理組織学的には全例腎細胞癌で，嚢胞壁より突出する腫瘍はhobnail cellを認めたため嚢胞壁より発生した腫瘍と診断され，一方，隔壁を有する腫瘍は多房性嚢胞状腎細胞癌と診断された。組織学的特徴および予後を考慮すると，積極的に腎保存手術を施行すべきであると考えられた。

We report 6 cases of cystic renal cell carcinoma (RCC).Tumor was considered to originate within simple renalcyst in 4 cases, and to be multilocular cystic type in 2 cases.All cases were diagnosed preoperatively as malignantneoplasms with radiological examinations,including CT,MRI and angiography. Radical nephrectomy were performedin 5 cases and partial nephrectomy in one case. Histologicaldiagnosis revealed that RCC occurred within simple renalcyst because of hobnail cells in the renal cystic wall in 4cases, and mulcilocular cystic RCC in 2 cases.
