健診を契機に発見された多房性囊胞状腎細胞癌

A case of multilocular cystic renal cell carcinoma detected incidentally by a routine medical checkup

症例は52歳，男性。健診の超音波断層検査で左腎腫瘤を指摘され当科を受診した。諸種画像所見より多房性囊胞状腎細胞癌もしくは多房性腎囊胞と診断。悪性疾患を否定できなかったため，経腹式左腎摘除術を施行した。病理組織学的に多房性囊胞状腎細胞癌（pT2N0M0，stageⅡ）であった。本腫瘍は比較的稀であるが，特に良性疾患である多房性腎囊胞との鑑別が難しく，泌尿器科臨床上，十分注意しなければならないと考えられる。

A 52-year-old man was found to have left renal tumor by the abdominal ultrasound at a routine medical checkup. Imaging studies confirmed the presence of multilocular cystic renal tumor. Differential diagnosis includes multilocular cystic renal cell carcinoma as well as multilocular cystic nephroma. Radical nephrectomy was conducted with the final pathological diagnosis of renal cell carcinoma,cystic type,clear cell,expansive growth,Infα,ly（-）,v（-）,G2,pT2N0M0. Awareness of this disease entity may at least help in preoperative differential diagnosis,management and patient consultation.
