尿膜管由来と考えられた膀胱印環細胞癌

Signetring Cell Adenocarcinoma of Urachus:A Case Report

　症例は，67歳，男性。肉眼的血尿を主訴に受診した。膀胱頂部に非乳頭状広基性腫瘍を認め，経尿道的膀胱腫瘍生検術にて印環細胞癌と診断された。頂部またはそれに近い前壁に腫瘍が存在すること，腫瘍と正常粘膜上皮の境界が明瞭であること，他の臓器には腺癌を認めないことより，尿膜管由来印環細胞癌と診断し，拡大膀胱部分摘除術を施行した。術後経過は順調で，腫瘍の再発，転移を認めていない。

A 67 year-old man was admitted complaining of macrohe-maturia. Cystoscopic examination revealed a non-papillary tumor at the dome of the bladder. Transurethral biopsy demonstrated signetring cell adenocarcinoma. Several examinations revealed no tumor in any other organ. Opera-tion was performed on October 1996. Histopathological examination demonstrated signetring cell adenocarcinoma originated from urachus. Our case is 7th in Japan, of sign-etring cell adenocarcinoma of urachus.
