陰部疼痛を主訴とした左精巣セルトリ細胞腫

A Case of Sertoli Cell Tumor of the Left Testis Presented with Pain in Scrotum

　49歳，男性。左陰嚢部痛を主訴に当科受診。20年前に左陰嚢の外傷の既往歴があった。超音波検査にて左精巣腫瘍を認め左高位除睾術を施行した。腫瘍は3.5×2.5×2.0cm大で割面は白色で一部出血を認めた。女性化乳房などの所見なく，各種画像診断にて転移を認めず，胚細胞性腫瘍マーカーはすべて陰性であった。病理所見はセルトリ細胞腫であり，周囲への浸潤はないが，脈管侵襲像および各視野1個の核分裂像を認めたため，悪性と診断した。追加治療は施行していないが，術後2年を経過した現在再発を認めていない。

A 49-year-old man was admitted with complaints of painin left scrotum. He had past history of injury in left testistwenty years ago. On examination, gynecomastia was notobserved. Ultrasonography demonstrated about 3 cm largewell circumscribed round mass at the left testis. With adiagnosis of left testicular tumor, left high orchiectomywas performed. The tumor sized 3.5×2.5×2.0cm and hadwhite cut surface with focal hemorrhage. Microscopically, the tumor was composed of restiform, circumvascular pat-terned short spindle cells with eosinophilic cytoplasms andmoderately atypical nuclei.
