前立腺肥大症に対する生体組織内レーザー凝固術（ILCP）の経験

Clinical Trial of Interstitial Laser Coagulation for Benign Prostatic Hyperplasia

　21例の前立腺肥大症の患者に対して生体組織内レーザー照射（ILCP）を施行した。穿刺回数は4〜16回（平均9.6回）であった。これらの症例について術前，3か月後，6か月後のIPSS,QOL,Qmax，体積および残尿量を評価した。3か月後で評価が可能な18例では，IPSS,QOL Qmax，体積，残尿量ともに全体の平均は有意に改善されており，6か月で評価が可能であった12例についてもすべての評価項目に関して術前と比較して有意に改善されていたが，3か月目と比較すると変化がなかった。合併症として尿閉2例，前立腺炎1例，後出血1例があり，再手術率は19％であった。以上より，ILCPは比較的安全な方法であり，適応を選べば有効な治療法であると考えられた。

From November 1996 to October 1997, 21patients with symptomatic benign prostatic hyperplasia were treated by interstitial laser coagulation of protate (ILCP). Mean age of patients was 71(48-83). Mean number of puncture was 9.6 times (4-16). Treatment was evaluated by IPSS,QOL, maximal flow rate, prostate volume and residual urine volume performed pre-operatively, 3 month and 6 month post-operatively. 12 patients who could be evaluated 6 month after ILCP showed statistically significant improve-ment. Of 21 treated cases, four patients required reopera-tion. No severe side effects were observed.
