80歳以上の膀胱癌の臨床的検討

STUDIES OF BLADDER CANCER IN PATIENTS OVER 80 YEARS

　80歳以上の高齢者の膀胱癌症例48例に関して臨床的に検討を行った。80歳以上で初診する症例は女性の比率が高く，初発症状として肉眼的血尿が最も多かったが，他の排尿異常を訴える症例も多かった。しかし，症状出現から受診までの期間が遅れる傾向はみられなかった。腫瘍の性状では多発性の腫瘍が多かった。治療は膀胱温存療法が施行された症例が多く，再発率は79歳以下の症例と比較して高かったが，表在性腫瘍に対するTUR-BT施行後の再発に限ってみると有意差は存在しなかった。また，浸潤性膀胱癌に対する癌特異的生存率も79歳以下と差がなく，80歳以上の膀胱癌ではQOLを考慮した膀胱温存療法の必要性が示唆された。

Clinical observation was made in 48 patients with bladder tumor whose age was over 80 at initial visit and compar-ed with bladder tumor patients under 79 year old. Male to female ratio of these patients was 2.5 to 1. Although most of the chief complaint of these patients were asymptomatic gross hematuria, other symptom as dysuria was more frequent than the yunger age group. A period from the onset of symptoms to initial visit was not differ-ent between the two groups.
