膀胱癌の動注併用一時的阻血療法

TREATMENT OF VESICAL CARCINOMA USING BALLOON CATHETER OCCLUSION OF THE INTERNAL ILIAC ARTERY

　膀胱癌37例について内腸骨動脈の血流を一時的に48ないし120時間遮断するという阻血療法を施行した．37例中36例は阻血療法後手術療法を追加施行した．1例は阻血療法のみで完全に腫瘍は治癒した．阻血療法にて末梢血のリンパ球が一時的に低下する例が見られたが，これは阻血療法にてリンパ球が腫瘍内に集合するためと推察された．一部の例では阻血療法前後に膀胱腫瘍の生検を施行した．そして腫瘍内および間質にリンパ球が集合することを観察した．しかし，リンパ球のサブセットの検索では特定のリンパ球が集合するのではなくすべてのリンパ球が集合するのが観察された．これらの結果から，阻血療法の治癒の過程にT細胞が関与していると推察された．

We investigated the efficacy of the treatment of vesical carcinoma by occlusion of the internal iliac artery using a balloon catheter. Following the insertion of a balloon catheter into the internal iliac artery, temporary block of the blood supply to the bladder was done for from 48 to 120 hours. 37 patients underwent this ischemic therapy. Thirty-six patients underwent the procedure preoperatively and the other one patient did not have an operation.
