表在性膀胱癌再発腫瘍に対する粘膜麻酔による外来でのcold-cup切除

Cold-cup Bladder Resection by Topical Mucosal Anesthesia for the Outpatients with Recurrent Superficial Bladder Cancer

　1994年6月から1997年8月までの期間に，横浜市立大学医学部附属病院泌尿器科において再発性の膀胱腫瘍21例（延べ32回）に対して2％キシロカインによる粘膜麻酔下にcold-cup切除および電気凝固による治療を行った。Cold-cup生検鉗子による切除時では88％，凝固時では72％の症例でまったく痛みがなかった。また，12症例に対して血清中のキシロカイン濃度を測定したが，すべてが測定限界値以下であった。また，Kaplan-Meier法による再発率は6か月で31％，12か月で62％であったが，単発で2mm未満の小さな腫瘍では，大きな腫瘍と比較して有意に再発率が低かった。したがって，このような腫瘍では外来治療が可能であると考えられた。

Between June 1994 and August 1997, 21 patients (32 total treatments) with recurrent superficial bladder cancer were treated at Yokohama City University Hospital. Under topi-cal mucosal anestheia using 2% xylocaine solution intravesi-cally, cold-cup resections and electrocoagulations were done. No pain were achieved 88% at cold-cup resection and 72% at coagulation. In twelve patients were examined serum concentration of xylocaine, which showed below lower-normal range in all patients.
