症例は73歳，男性。貧血精査中に腹部CTで右腎腫瘤が指摘された。MRIで下大静脈内腫瘍血栓が認められ，右腎腫瘍T3b NO MOの診断で根治的右腎摘除術および下大静脈腫瘍血栓除去術を施行した。病理組織学的診断は移行上皮癌であった。下大静脈血栓を伴う腎盂腫瘍は極めて稀であり，本症例は本邦報告8例目に相当する。
A-73-year old male was admitted to our institution becauseof diabetic nephropathy. Computed tomography, perfomledfor the evaluation of advancing anemia and microscopichematuria, Showed right renal mass. Magnetic resonanceimaging (MRI) revealed tumor thrombus extending intothe renal vein and the inferior vena cava. Preoperativediagnosis was renal cell carcinoma with vena cava thrombus. Right radical nephrectomy and vena caval thrombectomywere performed.
