骨転移を初発とし，停留精巣に発生した精巣腫瘍

A Case of Undescended Testicular Cancer Diagnosed by Bone Metastasis

　70歳，男性。腰背部痛を主訴に整形外科に入院した。脊椎MRIで第5腰椎に骨破壊像を，腹部CTで右下腹部巨大腫瘤，大動脈周囲リンパ節腫大を認めた。腫瘤の生検結果は精上皮腫であり，右陰嚢内容が欠如していたことから，停留精巣に発生した精巣腫瘍（精上皮腫，stage Ⅲc）と診断し当科に転科となった。化学療法を3コース施行した結果，縮小した原発巣に手術療法を，第5腰椎に放射線療法を行った。現在外来にて経過観察中である。

A-70-year old male was admitted with the chief complaint of lumbago. Imaging studies including CT scan and MRI revealed paraaortic lymph node swelling and L5 vertebral body compression suggesting metastasis, and huge right lower abdominal mass. His right scrotal content was absent. Histological diagnosis of the specimen obtained from the mass was seminoma. Based on the diagnosis of right un-descended testicular cancer, three courses of chemotherapy were performed.
