経直腸的超音波ガイド下に経尿道的切除術を施行したミュラー管嚢胞

Mullerian Duct Cyst Treated with Transurethral Resection Guided by Transrectal Ultrasound:A Case Report

　患者は60歳，男性。主訴は排尿障害。前立腺は小鶏卵大，表面平滑，圧痛はなく，経直腸的前立腺超音波（TRUS），MRIにて移行領域正中に直径2cm大のミュラー管嚢胞を認めた。TRUSガイド下に嚢胞壁および前立腺切除術を施行し，排尿状態は著明に改善した。排尿障害を伴う小さなミュラー管嚢胞に対して，TRUSガイドを併用することで経尿道的手術を安全に施行することができた。

A 60-year-old man visite our department complainingof mild dysuria and imaging examinations as TRUS, MRIan voiding cystourethrography disclosed a small Mullerianduct cyst localized in the prostate. Because of persistenturinary symptoms we performed transurethral unroofingof the cyst and resection of the prostate guided by transrectalultrasound for safety. Postoperatively he had no complaintof dysuria with marked improvement of IPSS and uroflow-metric evaluations.
