前部尿道に発生した尿道ポリープ

A Case of Anterior Urethral Polyp

　男性の尿道に発生するポリープは，比較的稀な疾患である。尿道ポリープは前立腺部尿道付近の後部尿道に多くみられるが，筆者らが経験した症例は，前部尿道に発生した非常に稀な尿道ポリープの1例である。症例は71歳，男性で，肉眼的血尿を主訴に1998年11月に来院した。精査の結果，血尿の原因は膀胱腫瘍と診断し，経尿道的膀胱腫瘍切除術を施行した。病理の結果，TCC G1＜G2 pTaであり，術後再発予防のために膀胱内へTHP＋Cylocideの注入を定期的に行っていた。1999年4月，再び肉眼的血尿を認めた。内視鏡上，前部尿道にポリープ状の腫瘍があり，経尿道的切除術を施行した。病理組織の結果，線維性ポリープと診断された。

A 71-year-old man visited our clinic complaining of as-ymptomatic macroscopic hematuria.He has been followedup with adjuvant therapy after transurethral resection ofbladder cancer, transitional cell carcinoma.An endoscopicexamination proved an oval tumor with stalk in the anteriorurethra and no relapse of bladder cancer. The tumor,10x0.5mm in size, was resected transurethrally and diagnosedpatho-histologically as a fibrous polyp. Though 19 casesof posterior urethral polyp have been reported previouslyin Japanese manuscripts, a polyp in the anterior urethrahas not been reported in the Japanese and English litera-tures.
