後腹膜腔に発生したガーゼオーマ

A Case of Retroperitoneal Gauzeoma

　腹部の遺残ガーゼは，生体の炎症反応によっていわゆるガーゼオーマを形成する。今回筆者らは，後腹膜腫瘤として発見されたガーゼオーマの1例を経験したので報告する。症例は66歳，女性，16年前に蛋白尿，尿潜血にて開腹腎生検を施行された既往があった。画像上では超音波検査にて内部が低輝度，一部音響陰影を伴った腫瘤を描出した。CTでは辺縁がやや高濃度で内部が低濃度であり，造影効果は乏しかった。良性腫瘍を考えたが，自覚症状があるため摘出術を行った。腫瘍は後腹膜腔にあり，一部腹横筋と癒着していた。病理検査の結果，線維性な部分と肉芽腫様の部分が混在している中にコットンファイバーと思われる線維性の異物像が散在性に認められた。16年前の開腹術の際に遺残したガーゼ線維によって発症したガーゼオーマと診断した。

A 66-year-oldwoman with a complaint of right abdominal dull pain andmacrohematuria, who had undergone open renal biopsy 16years earlier, was suspected to have a retroperitoneal tumorby sonography. CT scan showed a well defined round masswith a thick wall and internal heterogeneous density. Thetumor was extirpated by open surgery. Pathological find-ings revealed infiammatory granuloma with a small amountof cotton fibers.
