癌性疼痛に有効であった，クモ膜下繰り返し注入法によるフェノール神経ブロック

A Case of Intractable Cancer Pain Treated by Repeated Subarachnoideal Block with Phenol Glycerine

　膀胱癌の右坐骨，腰椎転移による疼痛患者に対し，モルヒネを投与したが除痛効果は不十分であった。硬膜外力テーテルの留置，放射線照射でもほとんど鎮痛効果は得られなかった。そこで，腰椎クモ膜下腔に7％フェノールグリセリン0.5mlを，10分間隔で3回に分けて繰り返し注入し，神経ブロックを行ったところ，ほぼ完全な無痛が得られた。副作用として軽度の便失禁と下肢麻痺が出現したが，極めて高い除痛効果を認めた。

A 60-year old woman with bone metastasis of progressive bladder cancer, who underwent total cystectomy and bilateral ureterostomys previously, was given oral and parenteral opioids for the severe leg pain, but it never stopped. A continuous epidural nerve block and radiation therapy were also ineffective. To eliminate the intractable pain, 0.5ml7% phenol-glycerin was injected into the lumbar subarachnoid space three times at ten minutes interval. The treatment provided rapid and long term pain relief. The adverse effects were slight leg paralysis or disturbance of bladder, rectal function.
