対側腎周囲脂肪組織転移をきたした腎細胞癌

Metastasis to contralateral prinephric fat of a renal cell carcinoma

　69歳，男性。1992年8月左腎摘除術施行。病理診断は淡明細胞癌，pT2N0M0であった。2000年9月腹部CTにて右腎周囲脂肪組織内に，長径4cmの孤立性腫瘍を認めた。同年10月腫瘍切除術を施行し，病理診断は淡明細胞癌であった。腎細胞癌の対側腎周囲脂肪組織への転移の本邦報告例は同時性のものが1例あり，自験例が2例目である。

　A case of renal cell carcinoma（RCC）,considered to be a metastasis to the contralateral perinephric fat,is reported. A 69-year-old man had undergone a left radical nephrectomy for RCC（pT2N0M0）. Eight years after surgery,computed tomography revealed a mass,4cm in size on the right perinephric fat. Metastasis of the RCC was suspected and removal of the mass was peformed. Histologically,the mass was identified as RCC metastasis. RCC metastasis to the contralateral perinephric fat is clinically rare.
