精巣上体転移を来した腎盂腫瘍

Renal Pelvic Tumor with Metastasis to Epididymis

　65歳，男性。左下肢痛を主訴に近医を受診し，当院整形外科を紹介され，左腓骨の転移性腫瘍と診断された。原発巣精査中に左腎盂腫瘍を指摘され，当科を紹介され受診となった。左側腹部に腫瘤を触知し，左陰嚢内容の腫大を認めた。組織学的診断のため左精巣摘除術を施行した。精巣上体の腫大を認め，病理組織診断の結果は移行上皮癌であった。本症例は，腎盂腫瘍の精巣上体転移の本邦2例目と考えられた。

Epididymal metastasis from transitional cell carcinoma of renal pelvis is very rare. We report a case who presented with a palpable mass of left scrotum and left knee pain. Computed tomography revealed a 8×7×7 cm mass within left renal pelvis. Left orchiectomy was performed and histopathological examination revealed epididymal tumor which was metastatic transitional cell carcinoma resulting from the renal pelvic tumor. Metastatic route was consid-ered as retrograde venous extension or embolism.
