恥骨後式前立腺全摘除術の検討—術前評価と病理診断との関連性を中心に

A Clinical and Pathological Study on Retropubic Radical Prostatectomy:Correlation between Preoperative Evaluation and Final Pathology

　最近4年間に恥骨後式逆行性前立腺全摘除術を施行した前立腺癌50例を対象として，その術前評価と病理学的根治性の関連を検討した。最近になるほど前立腺全摘例は増加しており，後半2年間にスクリーニング発見例が多かった。pT2以下の症例は28例（56％）であったのに対し，pT3症例では22例（44％）中6例（12％）で所属リンパ節に顕微鏡的転移が証明された。臨床的病期と病理学的病期の相関は，neoadjuvant非施行群では76％の一致率，neoadjuvant施行群では55％の一致率であった。また，針生検時の癌の分化度とsystematic biopsyにおける癌陽性core本数および治療前PSA値は，手術による根治性を予測する上で重要な所見と思われた。

The significance of anatomical radical prostatectomy was evaluated by analyzing the correlation between various preoperative findings and final pathology of resected pros-tate. During last 4 years,atotal of 50 patients with localized prostatic cancer underwent anatomical radical prostatec-tomy. 29 patients were treated with neoadjuvant hormone therapy. The final pathology of resected prostate showed that 28 patients (56%) had pathological stage below than pT2, while 22 patients was pT3 and 6 patients with pT3 had microscopic lymphonode metastasis.
