高齢者の術前リスク評価—POSSUM scoreの検討

Surgical Risk Assessment in the Elder Urologic Patient Using the POSSUM Scoring System

　高齢者開腹手術を安全に行い得るためのリスクをPOSSUMスコアを用いて検討した。対象は最近5年間に手術時間3時間以上の開腹手術を施行した75歳以上の高齢者50例で，合併症発症例が11例，非発症例が39例であった。合併症発症群は，身体スコアおよび手術侵襲スコアともに非発症群に比べて有意に高値であった（p＜0.05）。特に身体スコア22点以上の症例は術後合併症を発症しやすく，厳重な管理が必要である。高齢者に侵襲の大きな開腹手術の適応を決定する際，POSSUMスコアはリスク評価法の1つとして有用と考えられた。

We evaluated the risk of surgery in elder urologic patient using the POSSUM scoring system. In a recent 5-years period, 50 patients over 75 years old have undergone urologic surgery lasting more than 3 hours. We divided them into two groups, 11 patients with complications 39 patients with no complication. In the complication group, both physiological score and operative severity score of the POSSUM were higher than no complication group (p ＜0.05). In addition, the patients with more than 22 points in physiological score had more postoperative complica-tions.
