尿閉を主訴に発見された後腹膜線維肉腫

Retroperitoneal Fibrosarcoma with the Chief Complaint of the Urinary Retention

　症例は62歳，男性。尿閉を主訴に近医を受診した。骨盤CTにて膀胱直腸間に10×8cm大の腫瘍を指摘され当院紹介となった。骨盤MRIでは，筋組織とほぼ同信号を示す周囲臓器とは境界明瞭な腫瘍であり，血管造影ではhypovascularを示した。後腹膜腫瘍と診断し手術を施行した。病理組織検査では，異型性を示す核を有する紡錘細胞の束状配列を認め，免疫特殊検査においてvimentinのみ陽性を示したため高分化型線維肉腫と診断された。後腹膜を原発とする線維肉腫は稀であり，自験例は本邦報告26例目であった。

The patient was a 62-year old man who complained of the urinary retention. Radiological examination including computed tomography (CT) , magnetic resonance imaging (MRI) and angiography showed that the hypovascular tumor was localized between the bladder and the rectum. In June 1996, complete tumor resection was perfomed. In the hematoxylin- eosin preparation, the spindle cells were ar-ranged votically. Observing the immumo-special stained preparation, vimentin preparation was only positive. His-topathological diagnosis was fibrosarcoma.
