原発性上部尿路上皮内癌の1例

PRIMARY CARCINOMA IN SITU OF THE UPPER URINARY TRACT: A CASE REPORT

　75歳，男性。無症候性顕微鏡的血尿を主訴に来院した。静脈性腎盂造影，膀胱鏡において異常を認めなかったが，尿細胞診では，右分腎尿Class Ⅴであった。右腎盂腫瘍と診断し，右腎尿管全摘除術を施行した。病理組織学的には，右腎盂から上部尿管にかけて上皮内癌を認めた。術後，30か月を経過しているが，再発の兆候は認められていない。

A case of primary carcinoma in situ of the upper urinary tract in 75-year-old man is reported. The patient present-ed himself to our hospital because of microscopic hematuria. An intravenous pyelogram and retrograde pyelogram showed no abnormalities. Cystoscopy revealed no evidence of disease. But urinary cytology was positive in both voiding urine and catheter urine from the right pelvis. Right renal pelvic tumor was suspected and right nephroureterectomy was performed. Pathologi-cal diagnosis was primary carcinoma in situ of the right pelvis and upper ureter.
