自然破裂を来した腎細胞癌

Spontaneous Rupture of Renal Cell Carcinoma:A Case Report

　63歳，男性。結核性胸膜炎，転移性肺腫瘍を疑い精査・加療中，右側腹部痛が出現した。右腎周囲に血腫を認め，腎腫瘍を疑い右腎摘除術を施行した。右腎細胞癌の自然破裂であった。術後，INF—αを投与したが，術後7か月に死亡した。腎自然破裂は血腫のため鑑別診断に苦慮するが，原因は悪性腫瘍が30〜35％であり，悪性腫瘍が否定できない場合は腎摘除術が施行されている。腎細胞癌の自然破裂は，本邦報告例は23例であった。

A 63-year-0ld man was admitted to our hospital with the chief complaint of a severe pain in the right back. He had no history of trauma. Right perirenal hemorrhage was diagnosed on abdominal CT. Suspecting perirenal hemor-rhage caused by spontaneous renal rupture of renal cell carcinoma, we performed radical nephrectomy. Histopatho-logical examination revealed right renal cell carcinoma with perirenal hemorrhage. Including our case, 24 case of spon-taneous rupture of renal cell carcinoma have been reported in the Japanese literature.
