急性腹症を呈した特発性副腎出血

A CASE OF IDIOPATHIC ADRENAL HEMORRHAGE PRESENTING AS AN ABDOMINAL EMERGENCY

　65歳，男性。突然の左側腹部痛を主訴に当院救急外来受診。腹部CT上，左腎上極から前下方にかけて，腎周囲に広範に広がる腫瘤を認めた。その後の血液検査でヘマトクリットの低下を認め，左副腎からの出血が強く疑われたが腫瘍性病変の存在も考えられるため，経胸腹的に左副腎摘除術を施行した。切除標本の割面は器質化した血腫で，明らかな腫瘍の存在は認めなかった。病理組織学的検査でも，腫瘍性病変や出血の原因となるような血管異常などを認めず，特発性副腎出血と診断した。

A 65-year old man with the sudden onset of severe generalized left flank pain while staying home was refer-red to our hospital about 4 hours later. On admission, CT scan showed a huge, high density mass at the left adrenal portion. During the next few days, however, the hematocrit dropped from 33.5% to 26.8%, suggesting adrenal hemorrhage. But we could neither reach a final diagnosis nor deny the possibility of a malignant tumor. Therefore, left adrenalectomy was performed.
