前立腺粘液癌の1例

MUCINOUS ADENOCARCINOMA OF THE PROSTATE: A CASE REPORT

　症例は90歳，男性。肉眼的血尿，頻尿を主訴に来院した。前立腺腫瘍マーカーは正常，画像診断，直腸診上，前立腺癌が疑われた。前立腺生検では，細胞外にムチンを含んだ腺癌で，PAS染色は陰性であった。消化管．肺には原発巣．転移巣ともに認めなかった。LH-RHアナログによる抗男性ホルモン療法を開始したが，無効であり．前立腺は局所で増大，尿道直腸瘻を形成し、播種性血管内凝固症候群による消化管出血のため死亡した。

A 90-year-old man was referred to our hospital with the complaint of gross hematuria and frequent urination. On digital examination, the prostate was markedly enlarged and elastic hard. The serum prostate-specific antigen level was normal. The histopathological findings of the prostatic needle biopsy revealed an adenocarcinoma containing extracellular mucin. Cancer cells were present in the abundant mucinous substance which was stained with PAS, Alcian blue, and Muticarmine, but the PSA was negative by immunohistochemical staining. There was no evidence of primary or metastatic lesion in the gastro-intestinal tract and lung.
