粘液産生性前立腺癌の1例

A case of mucinous adenocarcinoma of the prostate

　症例は72歳，男性。無症候性肉眼的血尿を主訴に受診。直腸指診にて前立腺は弾性硬，圧痛なし。PSAは1.7ng/ml,PAPは0.9ng/mlと正常範囲内。CEAは6.9ng/mlと軽度上昇。CT,MRI，経直腸超音波検査にて前立腺背側に多房性嚢胞状腫瘤を認めた。嚢胞部の穿刺吸引細胞診および前立腺針生検を行った。嚢胞内容は血性粘液で，PSA染色陽性の粘液産生性腺癌であった。前立腺全摘除術施行。粘液産生性前立腺癌であった。

A 72-year-old man visited our department complaining of asymptomatic macroscopic hematuria. Transrectal digital examination revealed findings similar to those of benign prostate hypertrophy. PSA (1.7 ng/ml), PAP (0.9 ng/ml),γ-Sm (0.6 ng/ml), AFP (3 ng/ml), CA19-9 (31 U/ml) were within the normal limit, but CEA (6.9 ng/ml) was elevated. CT, MRI, TRUS revealed a multi-locular cystic mass in the posterior side of the prostate. We performed transrectal needle biopsy of the prostate. Histological examination showed PSA staining positive mucinous adenocarcinoma cells.
