慢性腎不全患者にみられた高脂血症治療薬による横紋筋融解症の2例

TWO CASES OF DRUG-INDUCED RHABDOMYOLYSIS WITH ANTI-HYPERLIPIDEMIC DRUG IN CHRONIC RENAL FAILURE

　症例は糖尿病性腎症で血液透析中の34歳の女性と，保存期慢性腎不全の42歳の女性。高脂血症治療薬bezafibrateの常用量の投与により四肢筋肉痛，倦怠感，クレアチンホスホキナーゼの異常高値などの横紋筋融解症がみられた。いずれの症例も薬剤の投与中止後症状の消失をみたが，症例2では腎機能の増悪をみて，約半年後に血液透析導入となった。腎不全患者への高脂血症治療薬投与には十分な注意が必要である。

We present two cases of rhabdomyolysis associated with the use of anti-hyperlipidemic drug,bezafi-brate.

First one is a 34-year-old woman having been treated with regular hemodialysis (HD) because of diabetic nephropathy and second patient is a 42-year-old woman in chronic renal failure because of chronic glomerulonephritis. They complained of pain in their extremities, after anti hyperlipidemic treatment by therpeutic doses of bezafibrate. Serum transaminase and creatine phosphokinase were highly elevated in two patient and myoglobinuria was shown in one. We concluded that they developed rhabdomyolysis.
