精巣欠損児における成長障害とその治療

ANORCHID BOY'S RETARDATION IN PUBERTAL SOMATIC GROWTH AND ITS TREATMENT

　幼小児期に両側の精巣を欠損した4症例を最長15年にわたって追跡観察した。その結果，これらの精巣欠損児では思春期成長促進現象が認められず，−2.0SD程度の低身長にとどまった。これらの症例に思春期スパートをおこし，身長を伸ばすためには，性ステロイドの少量投与が必要であった。男児であればテストステロンでよいが，女児として育てられている場合にはエストロゲンによっても成長を促進させることができた。このような経験から精巣欠損児に対する思春期成長促進療法として，性ステロイドの投与開始時期と至適投与量を明らかにした。

Four cases of juvenile bilateral testicular defects were subjected to a follow-up observation of the somatic growth over a period of up to 15 years. The results revealed lack of their pubertal spurt even in the lapse of puberty and their resultant stature as low as -2.0 SD. Small dosage of sex steroid was necessary for triggering their pubertal spurt and increasing their stature; testosterone will do for males, and when patients have been brought up as females, their growth can also be promoted by estrogen.
