対側副腎へ転移した腎癌

A CASE OF RENAL CELL CARCINOMA WITH CONTRALATERAL ADRENAL METASTASIS

　患者は5年前右腎腫瘍にて経腹的右腎摘出術を施行された68歳，女性。実兄も腎腫瘍で死亡している。定期受診時の腹部CTにて左副腎部腫瘤を認めたため入院。諸検査にて腎腫瘍の副腎転移または副腎原発腫瘍を疑われ左副腎摘出術を施行。摘出腫瘍は病理組織学的に以前摘出された左腎腫瘍と同一のgrade 2の腎細胞癌と診断した。術後経過良好にて退院し，術後15カ月以上経った現在，明らかな再発転移を認めていない。

A case of contralateral adrenal metastasis of renal cell carcinoma is reported. The patient is a 68-year-old female, who lost elder brother by the renal cell carcinoma. On follow up CT scan, taken 5 years after the right transabdominal nephrectomy, left adrenal tumor was found. Metastasis of renal cell carcinoma or primary adrenal tumor was suspected and left adrenalectomy was performed. Pathologi-cally the tumor was the metastasis of right renal cell carcinoma. Her postoperative course has been satisfactory with no evidence of recurrence over fifteen months.
